Referring

The East Midlands (North) CEW service accepts referrals from secondary and tertiary care clinicians based within its geographical region. This is Nottingham and Nottinghamshire, Derby and Derbyshire (excluding Chesterfield), Lincoln and Lincolnshire (excluding Grimsby and Scunthorpe). We ask that referrals come from a medically qualified clinician, due to the need to assess for complications of excess weight at the point of referral and, where identified, consider further investigations, management and parallel referrals.

Referrals can be made using the CEW Referral proforma (see Referral Form download on this page) or via a referral letter, ensuring that the information on the proforma is covered. Where insufficient information is provided, this may result in the referral being rejected or returned pending clarification.

Referrals are also accepted from primary care for patients based in Nottingham and Nottinghamshire. Referrals are accepted from clinicians who have overall responsibility for the patient’s clinical care, which may include advanced practitioners where they are able to assess for complications of excess weight and, where identified, consider further investigations, management and parallel referrals. Referrals should be made on the CEW Referral Form via F12.

Patients should remain under the care of the referrer during the time they are seen in the CEW service and would usually be expected to be discharged back to the referrer at the end of their intervention (typically after 12-18 months).
